You're doing fantastic. I only made two changes: 1. Light direction. I could see that you were aware of the strong light aimed at the patient's mouth but didn't use it as a unifying feature. If the light is directly over the patient's head and the viewer is above too with the light a few feet in front of them, then we'll see strong cast shadows just below the dentist's arm, the chin, etc. 2. Create overlaps to sell the foreshortening. Your foreshortening isn't necessarily bad. It works best, though, when you've made it a main feature of the composition. I made the raised calf overlap in front of the thigh, and I nudged the entire raised leg to the left for a slight overlap above the lowered foot. I also made the raised foot way bigger.
