What can I improve?

Battlefield medics

I feel like the vibe of the artwork is fun but feels very flat in some areas. Don’t know how to improve the shining from explosion either. Please let me know if anything you guys feel needs improvements. Had to cover areas which has blood stains due to censorship. Thank youuu
